1. Claimant’ statement of Total and Permanent Disability
2. Attending Physician's Statement (TPD)
3. All other relevant medical reports e.g. lab reports
4. Insurance Advisor Report on Claims
5. Copy of the Insured’s NRC
6. Copy of 1st Page of claimant’s BankBook.
7. Authorization Form (if any)
8. Policy Contract
